Annual Questionnaires & Supplements – PKF Mid North

To help you gather the required records and information, we need to prepare your Financial Statements and Tax Returns.

Please use the links below to download and complete the relevant Annual Questionnaires and Supplements (if applicable). The questionnaires can be printed and filled in using a pen, or they can downloaded as a PDF to be filled in using a PDF editor on a computer or tablet. We currently do not have online form functionality.

Keep this together with your end of year accounting records until you are ready to bring them into our office. If you are unsure about any of the questions, please give our office a call so that we can assist you.

• Emailed to reception@pkfmidnorth.co.nz, or
• Uploaded to your Xero software in ‘Files’ (please also email us to let us know you have uploaded to Files), or
